Standardization of Physician Documentation

The Standardization of Physician Documentation Task Group is reviewing documents used by physicians in Fraser Health with a view to standardizing these tools to support the delivery of the highest standards of clinical care. 

The task group provides support and leadership across programs for the developers of standardized documents.  Since these forms and clinical tools are developed by physicians within departments for use by physicians, the Task Group encourages input from all physicians through our website, Docs on Docs (user ID and password required).

Feedback for the forms that are listed here under review can be made through your departments or directly to Docs on Docs by emailing your comments or suggestions to Dawn.Drummond@fraserhealth.ca.

You may also submit new forms that have been approved for use by your departments and divisions to Docs on Docs for consideration. 

All forms and documents presented by Department and Division Heads will be considered by the task group for use across and within programs in FH.  The review process includes presenting finalized draft documents and forms to the clinical policy office, HAMAC, MSFAC, and CIEC. 

Forms that have been approved for use in FH will be reviewed and revised from time to time to ensure that they continue to support the highest standards of clinical care and quality. 

The current documents under review will be open for feedback until April 15, 2012.  Forms that have been accepted and complete the review period without further revision will be placed into Forms on Demand and become available for clinical use.
